Some errors can make it necessary to kill -9 mfp

Errors in communications with remote processes or threads that won't die make it necessary to kill -9 mfp in some cases. This is always a bug.

I need to do a structured failure-modes analysis for the app as a whole and identify what kinds of failures can cause this. Then it should be just a straightforward grind to try to catch all those cases.
