Field and Text Box Alignment

The fields and the text box for user entry associated to it must be aligned on the screen. Please refer screenshot.
Other Points to recitify - Tab movement should be to every field
Some Fields should have high and low values restrictions - Like Pulse, Blood Pressue etc (Need medical values)
