The idea of the Stem table is exists in Rabbit-In-the-Hat, but there is no support in the CDM repository. Add DDL for the Stem table and view definitions for the various CDM tables that source the Stem table.

See https://github.com/OHDSI/sandbox/tree/master/CDMv5 an example of the stem (event_combinded) table ddl and views that will populate the various event tables.
