We need an sql script to take a v5.3 or v6.0 CDM to v5.4

In general it would be helpful to have mappings from one version of CDM to another - if for say we have CDM 6.0 we cannot use the DataQualityDashboard (https://ohdsi.github.io/DataQualityDashboard/index.html) as it requires 5.3.1 or v5.2.2.

If we remap our CDM to v5.4 then this issue will still persist - so having some kind of flexibility to transform to between major versions of the CDM so that we can utilize different tools would be helpful.

@AnthonyMolinaro can you take a look at this? I know you have the script to go from v5.3 -> v5.4 in PR #469 . Can we also add a script from v6.0 -> v5.4?

@jkylearmstrong just as an FYI both Achilles and Data Quality Dashboard now fully support CDM v5.4
