Instructions to use

• Install XAMPP Download

• Download and place all the files in htdocs folder under xampp folder "C:\user\xampp\htdoc".

• Open localhost/phpmyadmin and import the docx.sql file in phpmyadmin.

• Create a new record every time a new patient visits

• Next time the patient visits just update the records and their record history will be stored in history tab.

• Enjoy Its done :D
