I think we probably need a little bit more here. Is the expectation this is temporary? That we'll have multiple participants in our SG meetings, and it's only when it comes to voting that we ensure it's one Member one vote? Is it a single additional individual, or multiple?

IMO, doesn't need to be temporary. Maybe allow both to participate in SG meetings (though I expect typically only one person will be present at a time). A single additional individual seems sufficient.

Chris, Tantek, and I discussed this in meeting. These are my notes based on that:

  1. We need to carefully check that everything related to decisions only counts SG Members.
  2. We discussed whether these should be like deputy editors or not and the feeling was that "alternate" is correct. I.e., they can stand in for an SG Representative but both are not present at the same time in a decision or meeting. The exceptions to this would be the communication channels such as the shared email address, Slack, and the GitHub team.

Chris and I will also check with our respective organizations to see if there's any further feedback.

I don't think we should list affiliation twice. I'm also not sure a separate bullet point is in order as we expect at most one Alternate. Perhaps after the Representative in parentheses?

You wrote that the SG Member has the ability to appoint the SG Alternate Representative, not the SG Representative. I think that's probably the way to go, but that does mean there's no need for "except".
