'Activity' is not accurate in ERPNext 15

Information about bug

I created a document 3 weeks ago , but in 'Activity' it's showing "edited 1 yr ago'' . This bug is seen in many sites of V15.

Sometimes , it's like .. when I attach a photo in User doctype , in every document , the activity is updated as 'some other user, say 'X' , has attached a image".

Please solve this as it's important to get the accurate "Activity"
